Definitions:
A drug formulary, or preferred drug list, is a continually updated list of medications and related products supported by current evidence-based medicine, judgment of physicians, pharmacists and other experts in the diagnosis and treatment of disease and preservation of health. The primary purpose of the formulary is to encourage the use of safe, effective, and most affordable medications.

Prior Authorization (PA):
Prior authorization requires the prescriber to receive pre-approval for a particular drug in order for that medication to qualify for coverage under the terms of the pharmacy benefit plan.
Quantity Limits (QL):
A set of rules that set quantity limits on the number of units (tablets, capsules, etc.) allowed over a certain period of time for safety and management reasons.
Specialty:
Specialty medications are differentiated from other drugs in that they typically require intense clinical monitoring to manage side effects, frequent adjustments in dosage, and/or specialized training for handling and/or administration.
